Creates links to Rally with the ticket title as a hover over description for issues that are linked in Github-like sites (including GHE). Primarily focuses on the Commit, pull request, and the Commits on Pull request, and releases areas of the site. Includes linking from within pr comments, and Issue tabs of a repo.

- No longer attempts to embed a link when a rally ticket id is already contained in a link
